Medical Billing - A Look At Notes

It’s a strange world we live in. We expect most things to be a simple matter of ABC. We don’t want to have to think. We don’t want to have to go outside the box to figure things out. We want it all laid out there for us in plain simple to follow steps. The medical billing world is no different. Medical billing persons don’t want to have to think. Just give me the info that I need to get the bill out and move on to the next claim. Unfortunately, there are times when it isn’t so nice and neat when it comes to submitting a claim to an insurance carrier, especially when you’re sending claim to a heavily regulated carrier like Medicare. Sometimes, the carrier is going to want some narrative explanation as to why this claim is being sent. This is where notes come into play.

A note, in medical billing speak, is not something you play on your Yamaha keyboard, or a post-it you slip to the person in the cubical next to you to meet you at lunch time for a smoke. A note is just that, a short narrative explanation that is given to the insurance carrier to help explain why a certain procedure was done and why it is being billed. More importantly, it explains to the insurance carrier why the claim should be paid in the first place.

But why are they needed? Why should you have to explain to Medicare why a particular claim is being billed? If a procedure is done, or a device is prescribed for a patient by a doctor, shouldn’t that be reason enough? Isn’t the doctor’s word law? Doesn’t he know best? Well, maybe he does as far as medicine goes. As to medical billing, that is a different story. See, while the doctor may feel that a certain procedure or device is needed for the patient, the insurance carrier may not feel the procedure of device should be covered. This is very common with some elective types of surgery where the patient’s immediate health is not an issue and the surgery is more for comfort or appearance. For example, in some cases, surgery for hemorrhoids is not covered under insurance, especially if it’s a particularly new procedure like IRC. In this case, a narrative note must be sent to the carrier to explain why this surgery is required and why it should be covered by insurance.

In most cases, these claims will eventually be paid. But there are those cases where even with a narrative explanation, the insurance carrier will deny the claim, even with the narrative explanation. In this case, there is always an appeal and review process, but these things take forever to get through.

The point to all this, however, is when a medical biller sends a claim for a particular procedure that specifically states that a narrative explanation is required, that biller better have one from the doctor or get one. Otherwise, the chances of that claim being paid are slim to none.

You Have to Be a Little Crazy to Make a Career Change to Your Dream Job

Regardless of what profession you are in, you may want to make a career change to something better particularly a dream job or unconventional business that you have thought about for some time. Remember that there is no such thing as a perfect job or business, however.

There will always be barriers and adversity to overcome in any dream career or business. Even so to the inspired and committed individuals of this world leaving an old familiar job for something new is more adventure than adversity. New careers come with new problems but they come with new opportunities and wonderful experiences as well.

Truth be known, to make a dramatic career change to that dream job you have to be a little crazy! Indeed, whatever new field you decide to enter in pursuit of real success without a real job, chances are someone is going to think that you are nuts.

Relatives, acquaintances, friends, life coaches even you may doubt that you have what it takes to make it. The good news is that if others and you yourself think that you are at least a little bit crazy, this is a good sign that you can attain creative success in a new field of endeavor.

Perhaps you are, in fact, a bit crazy. Not to worry this is even a better sign!

In his book, The Hypomanic Edge (Simon & Schuster, 2005), John Gartner establishes a link between craziness and success in America. Gartner, a psychologist at Johns Hopkins University, claims many of todays successful entrepreneurs and business people exhibit hypomania, an energetic and ebullient state, which is a milder form of the mania associated with bipolar illness. Moreover, he contends that leading figures in American history, including Andrew Carnegie and Henry Ford, had the condition as well.

While many successful entrepreneurs are not entirely crazy, they are not entirely normal, which leads others to believe they are crazy. Talk to Americas successful entrepreneurs and you will find that most of them had at least one friend or acquaintance even a spouse who thought that they were crazy to attempt what they did. Yet it was their willingness to be a little bit crazy that allowed these entrepreneurs to achieve creative success.

Allow me to use myself as an example. For years, I have made a living from working leisurely telling North Americans not to work so hard. To many people this sounds crazy and it is to a certain degree.

I don’t care, however. My career life is working just fine. I make an income in the range of $100,000 a year by working only when I want to work. Whats more, if I pursue a few more crazy ideas of mine, I am sure that I can increase my income to $300,000 to $500,000 a year in the next few years by just working four or five hours a day.

Best of all, I know that I am making a major contribution to people’s lives because of all the positive feedback via letters and phone calls that I have received from readers of my books. Compare my lifestyle to the lives of millions of reasonable people slaving away at conventional jobs and you will see much value in being different, even a bit crazy.

So allow the craziness in you to shine. Combine your craziness with courage, patience, and perseverance and you will find yourself at places you didn’t think were possible.

In short, if you dream about writing books, then crazily start writing books. If you dream about making a documentary about Thailand, then experience your craziness and hop on an airplane for Bangkok. And if you dream about being a visual artist, then start creating crazy paintings or drawings. Otherwise, you will find that the ache of not pursuing your dream career can be the biggest pain you can ever experience.

The Changing Face Of Direct Mail Recruitment

DIRECT MAIL RECRUITING TECHNIQUES

The process of direct mail recruiting has changed dramatically over the past years. The days when you could recruit medical clinicians by simply sending one or two direct mail pieces is not working anymore.

The medical staffing recruiting community has changed and evolved to the point htat the stakes have been raised and continue to be raised. Candidates have more choices than ever before (no only with agencies but medical facilities), and their access to information is greater than it’s ever been.

All of this put together, pushes the envelope to the point that it has created an environment that is increasingly more difficult for medical staffing agencies to attract and retain qualified medical personal.

THE COMPETITION

Today there are many more medical staffing agencies as there were five years ago. THe barrier to entry is set relatively low, because the cost of starting a medical staffing agency is comparatively low (unless you buy into the medical staffing franchises). Today, a medical staffing agency can afford to advertise and promote its company via the Internet and reach a larger pool segments than ever before.

THE PROBLEM

Althouth Internet marketing is a viable marketing source, it should not be the only medium you use to promote your medical staffing agency to recuit candidates. Direct mail is seen by many as a waste of money that may not produce results. This form of thinking works to your advantage and guess what, it still works.

Your competition may have increased and grown, there are still only a small number of medical staffing agencies that are still actively pursuing direct mail marketing.

SECURING NEW RECRUITS

It is a common truth that direct mail when done right will convey credibility and alert the reader to action. Using direct mail places your recruitment efforts to build a foundation of credibility and branding. Why? Because it is believed that direct mail is too expensive or ineffective to produce the desired results. Within the medical staffing agency your target market falls within a particular niche thereby casting a smaller net than say a generalist. THe direct marketing dollars is probably more effective and more cost effective thatn other advertising mediums.

You are able to target audience precisely and eliminate the waste associated with typical advertising expenses. If you are a small medical staffing agency looking to hire only medical professionals within a certain geographical area, you would be wasting your money if you are advertising in magazines that have a wider circulation.

Don’t favor one marketing medium over others. It is essential to function within multiple marketing mediums in order to strike a lasting branding effect and begin your recruiting efforts. The moment you stop marketing is the moment your recruiting efforts will be taken by your competition.

Other than your degree at (College or University) what else are you doing to secure your position in the industry?

While your degree is important, employers want to see that you have progressively moved into your career. Book knowledge is important as it sets the foundation, however, your participation in extra cirricular activities may launch you ahead of the competition.

An internship provides the student with an understanding of career demands and qualifications. Internships allow students to connect what they have learned in class with how it will be applied in a work setting.

Starting a Career in Nursing

Midlife career changes are nothing new. If youre looking for a field with abundant opportunity and if you enjoy caring for others, nursing might be right for you. The job outlook is robust: The Bureau of Labor Statistics reports employment among registered nurses will grow faster than the average for all occupations through 2008. Some reports indicate that more than 1 million new and replacement nurses will be needed by 2012. A career in nursing is also lucrative: Typical salaries start in the $35K-$50K range and increase significantly for more specialized positions. Some hospitals are even offering signing bonuses upwards of $14K. So if youve spent some time off from work or are looking at a career switch, following are a few tips to help get you started in a nursing career:

Do your homework. There are numerous specialties to choose fromincluding oncology, pediatrics, recovery, and emergency careand several settings in which you can work. Practices range from home care to private practice to clinics to hospitals. Determine your course by actively networking. Attend career fairs and educational sessions. Contact your state nursing board, the local nurses association, or the closest chapter of Sigma Theta Tau, the nursing honor society. The more informed you are, the better you can map out your direction.

Advanced Practice Nursing (APN)

Certified Nurse Midwife (CNM)

Certified Registered Nurse Anesthetist (CRNA)

Clinical Nurse Specialist (CNS)

Recognize the steps involved. According to the National Council of State Boards of Nursing, several steps must be taken before a nurse can enter the profession. This usually involves:

Graduating from a recognized nursing program

Meeting the specific requirements of the state board of nursing

Passing the National Council of State Boards of Nursing (NCSBN) NCLEX examination for registered nurses (RNs) or licensed practical/vocational nurses (LPN/VNs).

Carefully choose a school. There are over 1500 nursing programs in the country. The minimum education requirement to get into nursing programs is a high school diploma, but advanced degrees require higher education. Most require an entrance exam. There are three basic types of programs available:

For those with bachelor degrees, post-bachelor programs are available which build on your previous education and put you on an accelerated course. This allows you to finish in 12-18 months.

Bachelors of Science in Nursing (BSN degrees) are four-year programs at colleges and universities. They build a strong foundation, providing leadership and management skills.

Associate Degree in Nursing (ADN) are available at two-year programs found at many community and junior colleges. Pre-nursing courses are typically required.

The school you choose is vital. It will largely determine how prepared you are for the NCLEX. The schools you consider should be state board approved and accredited.

Know your states licensing requirements. Get all the specifics on state licensure including the license period, fees, and continuing education requirements. How to Ask for a Raise - and Get it

Getting a raise should be the first thing you think about when the
subject of increasing your income becomes important. Of all the
things you can do to better your monthly finances starting a
home business, getting a part-time job, studying for a degree, etc., having your current pay increased at your present place of employment
tops the list. It is the quickest and easiest of all your options. One
quick meeting with your employer results in hundreds of extra dollars
being deposited into your bank account every month!

So who should ask for a raise? Anybody who hasnt had one in a while,
is underpaid for their level of skill and/or experience, or has really been
going the extra mile in their job performance over any reasonable length
of time (weeks, not days). If your paycheck comes from the government,
you fall outside the realm of people that can ask for a raise, because raises
are dealt out in a pre-defined schedule. However, many governmental
positions assign the level of your raise according to a supervisors periodic performance review. Government workers would do well to pay attention to some
of the advice I am about to give, so they can nudge their next pay raise up a level.

The actual meeting where you put in your pay increase request doesnt
take very long. Its a mistake to focus on the meeting itself in your quest. Its
also a mistake to have the meeting spontaneously. You want to set that up
at the end of a carefully crafted campaign, which you implement over the
course of several weeks (or more). The meeting is the finishing touch to the process,
and if you set it up right, you will be walking into it with a professional level
of confidence.

Now, if you are thinking about coming in early with bagels and staying late
to finish up working on projects, you have the right idea. But much more
important is the visibility you have to your superiors in your actual work ethic.
That means getting your work done, and done well, and done quickly and then
getting with your superior to let them know you are finished and can take on more
work. But dont just do that also casually communicate (without making it
obvious) some issues that popped up and how you creatively solved them in the
process. You basically want to wow your bosses for a few weeks. Thats right,
you need to keep this up for several weeks straight!

If you belong to an office click, avoid hanging out with the gang during your
campaign. There might be a person or two in that crowd that your supervisors
arent crazy about, and you dont want to be associated with anybody. Stand out
as an individual, but of course still play well with others and be a team player.
Recruit allies in other departments, especially supervisors, if you work in the kind of environment where your boss will do some research before making a decision.

When you do have the meeting with your supervisor, dont let them know exactly
why you want to see them if you can help it. Otherwise they may creatively find
ways to put that meeting off, or prepare a reasonable rebuttal as to why the
company cant afford it at the moment. Ask for more responsibility while you are asking for your raise, but dont turn it into a promotion request only. You are there to get a pay increase that you already deserve for your high level of performance at your current position, which you have been absolutely cranking at lately.

Dont present your request as an ultimatum. Act like a company-loyal fixture that is in it for the long haul. Dont quote co-workers salaries or base you argument on what others are making. Let your work performance speak for itself. Do mention the low-income lifestyle your current pay is restricting you to, and that you feel you deserve better. (This is a very subtle way of suggesting you may have to seek employment elsewhere, even though you would hate to, if you are not properly taken care of.) If you do the setup well, you will control the meeting and your request should be a slam-dunk.

Positive work behaviors is by far one of the most straightforward tips I can give you when it comes to moving up the corporate ladder. Do you sometimes feel that whenever people speak about moving up the corporate ladder, it almost always has a cunning undertone?

For career newbies especially, my recommendation is to always focus on positive work behaviors that you can start practicing now. When the time comes, moving up the corporate ladder will seem effortless. This is because you do what is right and focus on the positive. Bosses will notice and when the right time comes, the promotion you want is just a natural next step.

So what are the 8 straightforward positive work behaviors for moving up the corporate ladder?

1. Ideas

Regardless of how junior you are in an organization, you can have ideas that positively affect the company. Do not be contented with just doing your own work. Do not be shy or embarrassed about sharing them either. Whats the worst that can happen? They laugh at it and reject the idea? Well, then you turn it into something positive. Ask why cant it work, and you would have learned something new. That information can be used to come out with better ideas.

2. Impact

Be smart enough to choose some projects in a year that are considered a challenge for your level of experience. Be hardworking enough to see it through. But have the wisdom to cross check with your bosses to make sure it is a challenge and not impossible for your experience. That way you can create an impact in the company. This positive work behavior ensures that you are not just the average worker clocking in and clocking out without making an impression.

3. Impeccable

This positive work behavior is to be flawless in your language and dressing. Know that you are judged at all times. First impression or not, people judge you on how you talk and what you wear. You are lucky if you work in a company with dress codes, then just dress up or down accordingly. Being impeccable when it comes to dressing does not mean expensive clothes. It simply means to dress well. Being impeccable with your language does not mean using bombastic words. Quite simply, it means abstaining from vulgarity.

4. Independent

Learn to work independently. In the age of empowerment, most employees are given the freedom to fail. Even if this culture is not practiced in your company, learn to work with minimal supervision. When you can work independently, it allows your boss to concentrate on other things. But remember to always update your boss on what and how you are doing. Working independently as part of positive work behaviors will help you in moving up the corporate ladder.

5. Ignite

Be an ignition of your company. Take the initiative to start something. It does not always have to be work oriented. It can be something fun or informal. For example, if you know some of your colleagues enjoy exploring different restaurants, why not start a group that does it after work Fridays? When you are a positive ignition, you can move up the corporate ladder quickly.

6. Interest

Show interest in your companys initiatives. Be an interested party in things involving your department. Some career newbies I know come across as being cynical with initiatives that are being carried out by the company. As part of positive work behaviors, show interest in these initiatives. Give it a chance and support them. Do not send signals that your bosses see you as being merely contented as being a passenger of the company. That does not help in your plan of moving up the corporate ladder.

7. Intangible

Do not always focus on the tangible benefits when working. There are a lot of values in doing things that at the moment does not seem to benefit you. For example, helping a colleague on a project. Volunteering to cover for someone whos sick. Taking on an extra project when your boss seems inundated with work. Positive work behaviors like these may not have immediate tangible benefits but in the long run, helps you in moving up the corporate ladder.

8. Integrity

This positive work behavior means to have a sound moral character. Practiced at its most basic level for a career newbies, it can be as simple as not talking bad about a colleague and gossiping. Do not be drawn into wrong doings like cutting corners. Be honest in your dealings with colleagues, clients, business partners and suppliers.

These positive work behaviors are straightforward and simple to practice. You need not have an evil plan when thinking about moving up the corporate ladder. Cliche as it maybe, the cunning plans makes you fall off the corporate ladder faster than you can climb it

Employees Or Independent Contractors For Business

A small business faces one or the other dilemma because of its limited resources and capital. At particular stages of growth, you may require an extra hand but find that your cost factor is restricting. You might think of hiring a contractor instead. Whether to hire a new employee or an independent contractor is an important business decision. Both options have their own separate pros and cons.

Hiring an Independent Contractor

1. Advantages

Reduced Overhead: This means reduced costs in expenses, payroll, benefits and other overheads. It also reduces stress on covering cost revenue for a new venture.

No Health Benefits: It reduces the high costs of employee health benefits that are a huge financial burden on small businesses.

Work on Demand: It offers flexibility to the changing work demands of your company. You can do without it during periods of slow growth period.

No Training Needed: The contract work force is fully trained and highly specialized.

2. Disadvantages

Lack of Control: The contractor has the choice of control over the job being executed.

Compensation Rates: These can vary for different projects and depend on the market demand.

Legal Aspects: Classify employees and contractors correctly to avoid liability of employment tax and penalties on account of misclassification of an employee as a contractor. Go by a simple rule of thumb. If you have the right to control the means and methods by which the work is accomplished, then the person is an employee. However, if you have right to control the results of the work, then the person is a contractor. Once the understanding of classification is clear, then hire either of them after considering their implication on your business contract.

Hiring Employees

1. Advantages:

Loyalty: Employees are more loyal and have greater commitment, resulting in greater productivity.
Flexibility. Willingness of the staff to take on additional roles gives you greater flexibility in running the business besides providing learning opportunities for the staff. It is also easier to meet deadlines with committed staff.

2. Disadvantages

Responsibility: The burden of supporting an employee through the payroll is greater.

Extra overheads: With an additional work force, besides the additional pay bill, you will require bigger space and equipment, etc.

Becoming a Manager: As the staff grows in number, you spend more time in managing them and your company gets more exposed to worker-related lawsuits.

Some Additional Tips

Whether you decide to hire a contractor or an employee, do remember the following:
If the payment of contractor is more than $600 annual, report the wages on IRS Form1099-MISC to authorities. Sign an agreement putting forth the terms of your business relationship. This should include type of services, time frame, payment details, confidentiality, warranty and other work-related information.

Both employee and contractor hire agreements should include the work/product/service copyright laws to guard against misuse or leaking out the proprietary information of your company.

Hiring an employee or an independent contractor has its pros and cons. Exercise the option that is most suited for your business. Do be careful about the legal aspects of both the options.

How To Be Successful At Work

What to do, if on the previous job you havent learned what is required from you on this one? The bitter truth of a life consists that amazing successes and greater talents often remain unnoticeable. That is why you should take care of yourself.

You get in two percent of the most successful applicants on the market, if:
- Youre in the top ten graduates of your university;
- You are a former well known sportsman or the known public person in the past;

4. Properly manage time. People that can complete work in time, conquer stress, and be creative at work are highly appreciated. Companies are constantly looking for those kind of people.

5. Negotiation skills. People that are capable of objectively and tactfully resolving problems dont need extra advertising.

6. Speech skills. People that are capable to convince individuals or groups of the listeners, always ready to listen and sympathize, and clearly express thoughts not only are very important to a company, but also have excellent chances for promotion.

7. Writing skills. People that are capable to create a nice report, memorandum, letter in simple and accessible language are always in great demand.

8. Organizing abilities. People that are capable to precisely define a problem and offer multiple ways of its solving, and choose from them the best one, they are not easy to find and they have a big chance to get a job.

9. Negotiation skills. People that are capable of analyzing information and defining solutions are very important for any company. The ability to ask the right questions and critically examine the answers will make a good impression on any employer.
